Youth Era Wraparound
Wraparound is a philosophy of care used to build support networks for young adults and their families.
What is Wraparound?
Traditional support models and systems of care typically place “experts” at the helm of youth and young adult services. The Wraparound method ensures that the young adult’s voice is the driving force of their support plan.
The process
The young adult, family members, natural supports, and local service providers work together to create a personal and family plan focused on community-based solutions.

The team creates an initial plan of care using a high-quality planning process that reflects the principles of Wraparound.
The plan is implemented, progress and successes are reviewed, and changes are made while maintaining team cohesiveness and mutual respect.
Plans are made for a purposeful transition from formal Wraparound to a mix of formal and natural supports in the community.
Wraparound Youth Partner
The Wraparound Youth Partner is key to ensuring high fidelity within the Wraparound process. The process is young adult and family driven, strengths-based, and solution-focused.
Youth Partners help young adults make educated decisions, lead their team meetings, establish and work toward life assignments, and receive one-on-one peer support while navigating youth-serving systems.
Our goal
If a plan is not meeting a young adult’s expectations or is inhibiting progress, the team reevaluates it so the plan more accurately fits their needs and goals.
